DRBFM

Design Review Based on Failure Modes (DRBFM) is a tool developed by the Toyota Motor Corporation based on the philosophy that design problems occur when changes are made to engineering designs. The idea is to make changes from a previous, proven design highly visible and explore concerns, causes and impacts of that change. The points of change, according to the DRBFM theory, are the source of future failures.

The basis for reliability is to not change the design. It is the equivalent of Just-in-Time in the manufacturing process. Disturbances of Just-in-Time in manufacturing are the increases or depletions to the planned inventory.
Mr. Yoshimura believes one can preserve reliability in a proven design by not changing it. If the design is changed, it should be done in small increments. The disturbance to a design is the discontinuity caused by a design change affecting the interfaces between parts. The design should not be changed simultaneously in two places. Making too many changes too fast will result in failures faster than our capacity to detect. The key is to make design changes visible. "
